Effective 5th of May 2010, only new visa application forms are accepted.
Please note, application must be filled either in English/Dutch/French/German language.
New Visa application form can be downloaded and printed from the link below. Forms are also available at the Visa Application Centre (free of charge).
Applicants have the option to select their preferred language (German, French or Dutch) in which they would like the Visa Application to be processed. The application will be assessed at the Embassy based on the language selected by the applicant on the visa application form at the time of submission. If the application form is submitted in English the application will be assessed at the Embassy based entirely on their discretion any of the languages (German, French or Dutch).
